Transfer Request
If you are a veterinary surgeon and would like to transfer a patient for overnight/ out of hours care please complete the form below, ensuring it is accompanied by the following documents:
- Full history.
- Lab results and blood work (please supply both normal and abnormal results).
- Radiographs or any images where applicable.
Please call on 01388 777770 to confirm that the patient can be transferred and to obtain an estimated cost.

- Please note that TRANSFER OF A PATIENT FOR OVERNIGHT/OUT OF HOURS care is NOT a referral. The patient will be under the care of our out of hours vet and the owner will be asked to collect their pet to transfer them back to your practice when you reopen.
If you would rather REFER the patient for ongoing investigation and treatment by one of our specific disciplines, please complete a referral form instead.
